DFA Passport on Wheels Schedule: San Juan City, Metro Manila

The Passport on Wheels schedule for this coming June 2018 will be at San Juan City, Metro Manila. The DFA Passport on Wheels will process PASSPORT APPLICATIONS in San Juan City on June 16, 2018 at San Juan Gym. Interested Applicants must submit their applications and COMPLETE REQUIREMENTS at the City Tourism and Cultural Affairs Office not later than May 30, 2018.

The DFA will process only 1,500 applicants on actual processing date, June 16, 2018.  STRICTLY NO WALK-INS.

Requirements for Passport on Wheels

I. New Application (Adult):

  • accomplished application form
  • original copy and photocopy of PSA Authenticated Birth Certificate
  • married females must submit original copy of PSA Authenticated Marriage Contract
  • original and photocopy of 2 Valid IDs

II. Renewal (Adult):

A. Renewal of Machine Readable Passports (Maroon), Green Passports, and any older passports: TREATED AS NEW APPLICATION

B. Renewal of ePassports:

  • accomplished application form
  • current ePassport with photocopy of data page
  • original PSA Authenticated documents that will support the change of name (if applicable): Marriage contract, Annotated Birth Certificate, Annotated marriage contract to show annulment/divorce, death cetificate of spouse

III. New Application (Minor):

  • personal appearance of minor applicant and either parent or authorized adult companion
  • accomplished application form
  • original and photocopy of PSA birth certificate
  • photocopy of passport or valid ID of either parent
  • school ID (if applicable)

IV. Renewal (Minor):

  • personal appearance of minor applicant and either parent or authorized adult companion
  • accomplished application form
  • original and photocopy of PSA birth certificate
  • photocopy of passport or valid ID of either parent
  • photocopy of current passport of applicant
  • school ID (if applicable)

Acceptable Valid ID’s for Passport on Wheels

  • Social Security System (SSS)/Government Service
  • Insurance System (GSIS) Unified Multi-Purpose
  • Identification (UMID) card
  • Land Transportation Office (LTO) Driver’s License
  • Professional Regulatory Commission (PRC) ID
  • Overseas Workers Welfare Administration
  • (OWWA) / Integrated Department of Labor and Employment (IDOLE) card
  • Commission on Elections (COMELEC) Voter’s ID or
  • Voter’s Registration Record (VRR) from COMELEC Head-office or Regional Office
  • Philippine National Police (PNP) Firearms License
  • Senior Citizen ID
  • Persons with Disabilities (PWD) ID
  • School ID (for students)

  1. I have question related DFA Dumaguete City,
    Why always full when I try to renew my passport upon my presence ,but when I talk to someone she wants to book my application for renewal,,is a FIXER allowed to do that?

    What we can do for this matters in DFA Dumaguete FIXERS ar allowed
