- Eligibility / License
- Photocopy of your CSC eligibility, professional license, or rating, if applicable.
- School Records (Photocopy of your Transcript of Records (TOR) and/or Diploma)
- Supervisory / Managerial Proof (for SG 24 and above only)
- Certification or official orders showing your supervisory or managerial experience, including the duration and duties performed.
