Civil Service Commission Regional Office IV (CSCRO IV) will start using the new system for online request for CSC eligibility records from 2010 onwards. This new system will start on October 1, 2018.

Procedure in Online Request for CSC Eligibility Records
Step 1
Client to accomplish the online form.
Step 2
Download, print and accomplish the Examination Records Request Form (ERRF).
Download, print and accomplish the Declaration Form.
Step 3
Upload the photo or scanned copy of the following:
- ERRF
- Declaration Form
- 2 Valid ID Cards
- PSA-issued Marriage Contract for women who married after taking the examination
- 1 x 1 picture with full name tag and signature (for certification of eligibility
Step 4
Pay the processing fee at any branch of Land Bank of the Philippines:
Note: Please be informed that the bank charges inter-branch deposit fee.
Step 5
Sent the original copy of the requirements as mentioned in Step 3 together with the original copy of official receipt issued by any CSCRO-4 Field Office or the bank validated deposit slip as proof of payment for the processing fee including the prepaid self-addressed return envelope from any courier of your choice to this address:
Step 6
Requested examination records and the official receipt will be released to requesting party within three (3) working days upon receipt of the complete documents mentioned in Step 3 and 5 using the prepaid self-addressed envelope.
