Section A: Student Information

Applicants must provide their:

• Full Name, Date of Birth, and Age
• Address, Barangay, and City/Municipality
• Contact Number, Facebook Account Name, and Email Address

Section B: School Information

You’ll need to write:

• School Name and Address
• Year Level and Semester/Trimester Enrolled
• Total Tuition Fee and Outstanding Balance

Section C: Parent or Guardian Information

If the student is a minor, a parent or guardian must provide:

• Full Name, Relationship to the Student, and Contact Number

Section D: Requirement Checklist

Applicants should attach certified true copies or clear photocopies of the following:

1. Certificate of Enrollment / Registration Form

2. Valid School ID (or Certificate of No ID)

3. Valid ID of Parent/Guardian

4. Government-issued ID of the Student

5. Billing Statement / Statement of Account

6. Screenshot or Copy of Academic Transcript / Online Report Card

Section E: Declaration and Consent

Applicants must sign to confirm that all details are correct and authorize PCSO and PACE to verify the information provided.
