2. Personal Data Sheet (Revised 2017; CSC Form No. 212)
3. Birth Certificate (PSA copy with receipt)
4. Certificate of Employment (if applicable)
5. Photocopy of two (2) Government-issued ID with picture
6. Certificate of Eligibility
7. Diploma/Transcript of Records (authenticated copy)
8. Related Training Required by the Technical Units concerned
9. Undertaking of Non-Pending Case (if presently employed)
