The PNPA BMI Form is one of the requirements for the Philippine National Police Academy (PNPA) Cadet Application. Deadline of application is until October 15, 2025.
Applicants need to download and print out a copy of the PNPA BMI Form containing their height, weight, gender and BMI to be filled out and certified by a licensed Physician (Government/Private).
